This course is intended for community clinicians who create and modify care plans as part of their clinical practice. 

By the end of this course, learners will be able to:

• Describe the key components of an interprofessional care plan and the care planning process. 

• Differentiate between Multidisciplinary and Interprofessional approaches to care.

• Define the first two core functions of the Most Responsible Clinician in relation to care planning.

• Relate the importance of interprofessional care planning to their care of clients.
