In this online course on Paper Documentation of Goals of Care, VCH health care providers will learn about the benefits of centralized and interdisciplinary documentation of Goals of Care, and will become familiar with the Advance Care Planning & Goals of Care Discussion Record, including how to access the form, what to include in their Goals of Care documentation, and where to place this form in the patient's paper chart. Opportunities to use this documentation record to improve the flow of Goals of Care information will also be explored.  (It is recommended that you use Google Chrome as your browser).
