This course is intended to provide members of the Kidney Care Clinic's interdisciplinary team with an introduction to "What Matters" Conversations (or Goals of Care conversations). Clinicians will learn the relevance of these conversations in caring for patients with chronic kidney disease, and will explore the benefits of these conversations to patients, families, and clinicians themselves. Conversation strategies and evidence-based language will be introduced to assess patients' priorities, wishes, and overall understanding of their illness and care options. Lastly, the course will provide direction as to how and where to document these conversations in CST Cerner.
